Form: Non-Standard Computer Request

First, obtain approval to order a non-standard computer from your Budget Unit Head (BUH) and the BUH supervisor, if required.  Second, decide the computer specifications (advanced computer as recommended by Gallaudet Technology Services or other).  Third, complete the form below. 

Complete the form below to request:

1. the recommended advanced computer or a computer with specifications needed for your unique academic or research needs.
2. hardware parts other than that described in the standard computer specifications.

The form must be completely filled out before submitting.  Put down either "N/A" or "Not Applicable" in empty box(es).
