Class Approval Information: This document contains information for faculty members who plan to require their students to run a research study as part of the course requirements. 

The Gallaudet University IRB Application

IRB Application: This is the official IRB application that must accompany all new proposals  Note: You will need the most recent edition of the Adobe Reader to view this document correctly.

*** If you are a Mac user, you need to right-click on the PDF file and select "Open with" and then select Adobe Reader to open the file. *** If you use PDF preview, it WILL NOT save your data as you send it to IRB.
