Checklist, Timeline, and Computer Account Activation
Print out the following documents to keep track of all important dates and deadlines and to activate your computer account.

Forms and Fees
Fill out the following forms online in your Bison Account except where indicated. Read the Bison student tutorials for instructions on navigating through your Bison account.
- Enrollment Process
- Intent to Enroll Form - $100 admissions fee
- Housing Contract - Fill out if you plan to live on campus; $200 housing deposit
- Meal Plan - available for selection only during online business registration. Check the "Timeline Chart & Calendar" above for business registration dates.
- Medical History Form (paper copy option)
- Health Insurance Waiver Form
- Intent to Enroll Form - $100 admissions fee
- Program Application/Forms/Registration
- Honors Application (by invitation only through letter of acceptance)
- JumpStart - Fall only, $325 per person; fill out in your Bison account
JumpStart: American Sign Language (optional)
JumpStart: Academic Success (required if it says in your letter of acceptance) - Pre-Participation Evaluation Forms (for athletes participating in sports)
- Family Orientation Registration (pdf) - Fall only
- Students with Disabilities Intake Form - required if you need special accommodations

Apply for PIN ID and insert Gallaudet University School Code -- 001443 - IFAA (Gallaudet University Institutional Financial Aid Application)
Before you can apply for IFAA in your Bison account, you must first submit your Intent to Enroll form along with the enrollment fee.
