Adult Degree Program Forms & Fees Checklist
Complete the following three steps: Step 1:
Submit the following mandatory forms and two fees as soon as possible.
Medical form required only if you live within 50 miles from Gallaudet University or plan to take a course at Gallaudet campus. If you are unsure, it is better to complete the medical form. Make an informed decision about purchasing a medical insurance policy. Please read Health Insurance Information and follow instructions carefully.
(Note: Students are automatically billed the cost of an insurance policy of more than $1800 per year if a waiver and proof of insurance are not submitted.)
Admission Fee: $50.00
Online Student Orientation Fee: $50.00
Step 2: Activate your CAMPUS and GDOC computer accounts.
Activation access begins after the Intent to Enroll form and the combined $100 Admissions and Online Student Orientation Fees have been received and processed. See the “Gallaudet User Accounts” for further instructions.
Step 3: Print your Enrollment Verification (Student ID) and use it to get student discounts. (Keep it in your wallet or purse or send it to businesses that offer student discounts.)
Follow these steps:
1. Log into bison.gallaudet.edu
2. Click on Self Service
3. Click on Student Center
4. Click on "other academic..."
5. Select Enrollment Verification
6. Uncheck all the boxes (Make sure your Academic Institution is Gallaudet University)
7. Click on SUBMIT 6. Click on Printer Friendly Version 7..... and print.
