Scheduling and Forms

The first step in the scheduling process is to submit an email request for an ASLPI evaluation to:  Please include your name and for which program/purpose you are taking the ASLPI. You will be guided through the steps of the scheduling process.

If you are paying via check, make your check payable to Gallaudet University. Forms must be received at least one week prior to the interview date. They can be sent via fax, scanned to email or mailed to the address below.
