2014 Benefits Open Season

The 2014 Benefits Open Season will run from November 10, 2014 through December 8, 2014. Below is a summary of benefits you may change during Open Season.  You can find more information on each benefit plan by visiting the Gallaudet University Benefits website.  Unless otherwise noted, you should submit the enrollment forms to College Hall, Room 114.


Health Insurance (FEHB)
 
·        To view the 2015 benefits plan brochures, you can visit this link.  You may also visit this link for the plan comparison charts. 
·        To enroll, cancel, or change your health insurance plan, please complete this form.  The form may be submitted through email (must be scanned) or delivered to College Hall,   Room 114.
·        The effective date of any change will be January 11, 2015.  It will affect your paycheck on February 3, 2015.

 
Dental Insurance (Delta Dental and FEDVIP) 
·       
You can only choose one dental plan.

·        To enroll, cancel, or change your Delta Dental insurance plan, please complete this form
·        To enroll, cancel, or change your FEDVIP dental insurance plan, please visit the BENEFEDS website.
·     If you are switching from FEDVIP to Delta, you MUST cancel your FEDVIP dental insurance plan through the BENEFEDS websiteIf you are switching from Delta to FEDVIP, you must cancel your Delta Dental insurance plan.  The form may be submitted through email (must be scanned) or delivered to College Hall,   Room 114.
·        The effective date of any change will be January 1, 2015.  It will affect your paycheck on February 3, 2015. 
 
Vision Insurance (FEDVIP)
·       To view the 2015 vision plan comparison sheet, please visit this link.  It is available on the third page. 
·        To enroll, cancel, or change your FEDVIP vision insurance plan, please visit the BENEFEDS website.
·        The effective date of any change will be January 1, 2014.  It will affect your paycheck on February 3, 2015. 
 
Long-Term Disability Insurance (LTD)
·        The biweekly premium is $0.08 per $100 of your monthly base salary.  This is deducted from your paycheck on a pre-tax basis.  Detailed information on LTD can be found on this link.
·        If you wish to opt out of LTD, you must complete the waiver form during Open Season.   The form may be submitted through email (must be scanned) or delivered to College Hall,   Room 114.
·        If you wish to re-enroll into the LTD program, you must provide evidence of good health.  This can be done outside of Open Season. 
·        The effective date of any change will be January 1, 2015.  It will affect your paycheck on January 20, 2015.

Flexible Spending Account (FSA)
·        You may contribute $100 to $2,550 towards Health Care and from $100 to $5,000 towards Dependent Care FSA in 2015.  The contributions will be deducted from your paychecks on a pre-tax basis.   
·     Effective January 1, 2015, qualifying partcipants can carry over up to $500 of unused 2015 health care funds into the next plan year.  Furthermore, participants will have only have from January 1, 2015 to December 31, 2015 to incur eligible expenses from their 2015 account.  The health care FSA account will no longer have a 2 1/2 month grace period.  
·   There is no carryover for dependent care FSA account.  However, the grace period still applies, meaning you have from January 1st to March 15th of the following year to incur expenses towards your prior year account. Note, you still must submit all expenses no later than April 30th of the next year.
·        To enroll or re-enroll, please visit the FSA website.  If you are currently participating in the 2014 plan year, you MUST re-enroll online if you wish to continue in the 2015 plan year.
·        The effective date of any change will be January 1, 2015.  It will affect your paycheck on January 6, 2015.

Flexible Premium Plan (FPP)
·        Your health and dental insurance premiums can be deducted on a pre-tax basis if you are enrolled in FPP. 
·        There is no additional cost for enrolling in FPP.  It just lowers your taxable income, thus enabling you to bring home more money.
·        To enroll, please complete this formThe form may be submitted through email (must be scanned) or delivered to College Hall,   Room 114.
·        If you were hired on or after January 1, 2012, you were automatically enrolled into FPP.  However, you were given an opportunity to waive FPP if you wanted to opt out.
·     The effective date of any change will be January 1, 2015. It will affect your paycheck on January 6, 2015.
