To help you become more familiar with the assessment process, here is some important information.

1.      The first step in getting assessment services is to complete the client information form and other paperwork.  You will NOT be adding to our wait list until we obtain the completed paperwork.

2.      Coordinator of Services or a graduate assistant will contact you to schedule a screening appointment.  The screening appointment is 50 minutes.  During the interview, you will discuss...

         i.  Why do you want an assessment?

ii.  What are the problems you are experiencing?

         iii.  Background Information

iv.  Decide how you will pay for the assessment (VR or self-pay). If you choose VR, bring the contact
information to the appointment so that Ms. Shird can contact your VR counselor to get approval for
              payment.

3.      You will be assigned to an assessment clinician after the payment is received and when there is an opening.  This person will contact you to set up your first assessment appointment.  The assessment will require 10-12 hours of testing (about 3-4 appointments).

4.      After testing is finished, you will be contacted when the report is ready which may take 3 or more weeks.  You will then schedule a feedback session with the clinician.

To help you become more familiar with the Mental Health Center (MHC), here is some important information.

1.      The first step in getting individual and/or couple counseling services is to make an appointment for an intake with the MHC secretary. The intake is a 50 minute interview between you and a counselor.  In this interview, you will explain why you are coming for counseling; provide the counselor with important background and family information, and ask the counselor any questions you might have.  The counselor will then write an intake report and the client assignment staff will decide which counselor best matches your needs.

2.      The MHC provides individual and couple counseling in which you meet with your counselor for 50 minutes once a week.

3.      If you feel you are in a crisis condition (e.g. having thoughts of hurting yourself, feeling out of control, having recently gotten some shocking news) a MHC staff member can meet immediately with you to help you get the appropriate treatment. Please see the secretary for a crisis intervention appointment.

4.      After the intake, you will be sent an email from your assigned counselor to schedule the first appointment.  When demand for counseling is high you may be placed on a waiting list for counseling service.  If this happen, the MHC client assignment staff will let you know through an email.

5.      Any information shared between you and your counselor, either in individual or couple counseling, is kept confidential.  No one can get that information without your written permission.  For clinical supervision purposes, cases are discussed only between the counselor and the MHC professional staff, when necessary.  The privilege of confidentiality is lost when you threaten to do harm to yourself or to another person.  Also, in cases where children or elderly people are in danger of abuse, information must be reported in the State in which it happened because it is required by law.

6.      The MHC provides psychiatric services on Tuesdays and Wednesdays only to students who are receiving counseling services at the MHC.

If you are receiving psychiatric services from the MHC, you are required to meet with a counselor regularly.

OFFICE OF STUDENT CONDUCT (SCREENING SERVICES)

  Alcohol Screening

To help you become more familiar with the OSC screening process, here is some important information.

1.      The first step in getting OSC screening services is to make the appointment with the MHC secretary by informing her that you need an OSC screening appointment. The appointment is $50.00 and consists of a 50 minute interview between you and a counselor.  In this screening appointment you will complete some paperwork and discuss the violation to determine your risk level.

2.      If you feel you are in a crisis condition (e.g. having thoughts of hurting  yourself, feeling out of control, recently received some shocking news) a MHC staff member can meet immediately with you to help you get the appropriate treatment. Please see the secretary for a crisis intervention appointment.

3.      Any information shared between you and your counselor, either in individual or couple counseling, is kept confidential except for documentation of required screening appointments, recommendations, and treatment compliance, attendance, and incident reports.  No one can get that information without your written permission.  For clinical supervision purposes, cases are discussed only between the counselor and the MHC professional staff, when necessary.  Confidentiality is lost when you threaten to do harm to yourself or to another person.  Also, in cases where children or elderly people are in danger of abuse, information must be reported in the State where it happened because it is required by law.

4.      At end of the screening, you will receive a letter stated that you have completed the screening.  You are expected to give the letter to the OSC.
