The Ontario Student Assistance Program (OSAP) is a combination of grants and loans for eligible students from Ontario, Canada, to provide them tuition assistance at Gallaudet. Before OSAP assesses an applicant's eligibility for funding, Gallaudet is required to complete a document providing OSAP information about education-related costs using the Program Information Form (PIF).
This document outlines procedures for the PIF and provides points of contact for assistance with filling out the OSAP application form and subsequent procedures that need to be followed.
The PIF for the current academic year is available on the OSAP website, https://osap.gov.on.ca/OSAPPortal.
Student applicants must do the following:
- Fill out the "Student Information" section. ("Student number" is the Gallaudet University student identification number.)
- Once a student has completed this section, send the form to:
Office of the Registrar
Washington, DC 20002-2695
Office of the Registrar's Responsibilities:
The Office of the Registrar has been designated as the official entity for handling all PIFs at the University. Student Financial Services will provide an accompanying estimate of the cost of enrolling at Gallaudet for the current academic year.
Upon receipt of a student's PIF, the Office of the Registrar is responsible for completing the following sections of the PIF:
- Institution information
- Program Information. The cost to attend Gallaudet for the current academic year will be annotated in United States dollars ($).
Completed PIFs will either be:
- Mailed through the United States Postal Service to OSAP's point of contact, Ms. Sherry Britton, Thunder Bay, Ontario, or
- Scanned into PDF and emailed to Ms. Britton.
