Office for Students with Disabilities
If you have a special need and require accommodations, it is your responsibility to provide documentation from an authorized individual (medical professional, psychologist, etc.) indicating the accommodations (low vision interpreting, tactile interpreting, visual aids, mobility orientation etc.) that you need while here at Gallaudet University.  Neither the Office of Students with Disabilities (OSWD) nor the Department of Public Safety (DPS) can provide services without this documentation.  In order to facilitate a seamless delivery of accommodations and services, we ask that documentation of special needs be submitted to SWD prior to your arrival on campus.  The application can be found online at oswd.gallaudet.edu/oswdforms.xml.

Library
Summer programs students have access to the Gallaudet University Library, which maintains one of the most comprehensive collections of deaf-related materials in the world along with a large collection of general materials that support the University curriculum.  The Library will be closed on May 28 and July 4, 2012.

Student Health Service
All students attending classes on campus must provide a completed Medical History Form, a Tuberculosis Risk Assessment Form or tuberculin skin test within 12 months prior to attending Gallaudet University (the Tuberculosis Risk Assessment Form will let you know if the test is needed), and documentation of immunization from Tetanus/Diphtheria. In addition, the District of Columbia law requires that all students under the age of 26 provide documentation of immunity from Measles, Mumps, Rubella, and Varicella. Students under 18 years must also be vaccinated against polio. It is mandatory that you receive your immunizations before beginning your class. Current Gallaudet graduate and undergraduate degree-seeking students should already have a Medical Health History Form on file. Questions about immunization requirements can be directed to shs@gallaudet.edu
