How to Request Transcripts

Complete the Transcript Request Form

Either pick up a Transcript Request Form from the Registrar's Office (Chapel Hall, Room 101) or download and print the form. You will need to complete the form with the following information:

• Your full name (include name changes)
• Full current address
• Day telephone number
• Email address, if available
• Social Security Number
• Date of Birth
• Student ID Number, if available
• Date(s) of attendance
• Complete address where transcripts should be sent
• Type of transcript wanted (official or unofficial) and the number of each
• Payment information

Cost

Unofficial transcripts are $1.00 each. Official transcripts are $5.00 each. You can pay by check, money order (payable to "Gallaudet University", or credit card (Visa or MasterCard). The cost of the transcript covers the cost of mailing the transcript by first-class through the U.S. postal service. If you would like your transcript faxed there is an additional fee of $10. Please note, however, a faxed transcript will likely be considered "unofficial?by the receiving institution or business.

Official vs. unofficial transcripts

An official transcript is a complete print out of your academic history and is printed on formal paper stock with the Gallaudet University seal, and the signature of the University Registrar. Many times other institutions of higher education or employers will require official transcripts. An unofficial transcript is a complete printout of your academic history at Gallaudet University, but it is printed on regular paper stock and doesn't come with the official University seal or signature of the Registrar.

Please note:
