The Academic Affairs / Office of the Provost is pleased to provide the following resources.

The below documents are accessible to users with a University account and are mostly for Faculty. Adobe Sign is the preferred way for you to submit forms, and the OneDrive documents should only be used for forms that are not available on Adobe Sign.

Please review the following instructions below before using Adobe Sign.

You will find all appropriate forms on your Adobe Sign Account. Refer to the Dashboard and the screenshot below to find shared documents. The appropriate documents are titled accordingly with "Academic Affairs" as a prefix.

These will be removed soon, once all forms are available within Adobe Sign. This includes the below PDF / Word files:
