Process to Register and Establish Eligibility

Complete Intake Form

Fill out online and submit the "OSWD Intake Form" (eSign), or come to the OSWD office for a copy of the form to complete.

Provide Documentation

Check the Documentation Guidelines to see what you need to document your disability, collect the information, then mail it, fax it, or bring it in person to the OSWD office.

Make Intake Appointment with Disabilities Counselor

The counselor will go over your intake form and all your documentation, and discuss the nature of your disability and your requested accommodations with you.

Receive Eligibility Determination

After you have been notified of your eligibility in writing, you are considered registered with OSWD, and you are considered a documented student with a disability at Gallaudet, and entitled to receive accommodations and services.
