2012 Open Season
The 2012 Benefits Open Season will run from November 12, 2012 through December 10, 2012. Below is a summary of benefits you may change during Open Season. You can find more information on each benefit plan by visiting the Gallaudet University Benefits website. Unless otherwise noted, you should submit the enrollment forms to College Hall, Room 114.
Health Insurance (FEHB)

· To enroll, cancel, or change your health insurance plan, please complete this form.
· The effective date of any change will be January 13, 2013. It will affect your paycheck on February 5, 2013.
Dental Insurance (Delta Dental and FEDVIP)
· You can only choose one dental plan.

· To enroll, cancel, or change your Delta Dental insurance plan, please complete this form.
· To enroll, cancel, or change your FEDVIP dental insurance plan, please visit the BENEFEDS website.
· The effective date of any change will be January 1, 2013. It will affect your paycheck on January 22, 2013.
Vision Insurance (FEDVIP)

· To enroll, cancel, or change your FEDVIP vision insurance plan, please visit the BENEFEDS website.
· The effective date of any change will be January 1, 2013. It will affect your paycheck on January 22, 2013.
Long-Term Disability Insurance (LTD)
· The biweekly premium is $0.08 per $1,000 of your base salary. This is deducted from your paycheck on a pre-tax basis.
· If you wish to opt out of LTD, you must complete the waiver form during Open Season.
· If you wish to re-enroll into the LTD program, you must provide evidence of good health. This can be done outside of Open Season.
· The effective date of any change will be January 1, 2013. It will affect your paycheck on January 22, 2013.
Flexible Spending Account (FSA)
· You may contribute up to $2,500 towards Health Care FSA or up to $5,000 towards Dependent Care FSA in 2013. The contributions will be deducted from your paychecks on a pre-tax basis.
· To enroll or re-enroll, please visit the FSA website. If you are currently participating in the 2012 plan year, you MUST re-enroll online if you wish to continue in the 2013 plan year.
· The effective date of any change will be January 1, 2013. It will affect your paycheck on January 8, 2013.
Flexible Premium Plan (FPP)
· Your health and dental insurance premiums can be deducted on a pre-tax basis if you are enrolled in FPP.
· There is no additional cost by enrolling in FPP. It just lowers your taxable income, thus enabling you to bring home more money.
· To enroll, please complete this form.
· If you were hired on or after January 1, 2012, you were automatically enrolled into FPP. However, you were given an opportunity to waive FPP if you wanted to opt out.
· The effective date of any change will be January 1, 2013. It will affect your paycheck on January 8, 2013.
Standard Life Insurance
· The Standard life insurance will be covering domestic partners as of January 1, 2012. If you currently have the Standard Life Insurance plan, you can purchase the dependent life insurance to ensure that your domestic partner is covered.
· There is no need to show evidence of good health if the enrollment form is submitted during Open Season. If the form is submitted after Open Season, then your domestic partner would need to provide evidence of good health.
· You may elect for dependent life insurance in multiples of $5,000 from $5,000 to $25,000.
